Most of the personal information processed by North Wales and Chester Dyslexia Assessments is provided directly by you via the following mediums:

  • Request for information via email or phone

  • Questionnaires

  • Previous assessment reports

Personal information may also be received indirectly, with your consent, from the following sources in the following scenarios:

  • Information provided by schools/other agencies (where applicable and with your written consent) in order to complete an assessment of your child.

We use the information you have given us in order to respond to your enquiry/booking request, action bookings and complete an in-depth, diagnostic assessment for you / your child (as applicable) where an assessment has been purchased.

How your information is stored:

Your information is securely stored by encryption and/or passwords and all assessment reports will be password protected when sent by an email. All physical documents will be stored in a locked filing cabinet in a secure location.

When you submit information to this website via webform, it is shared with Squarespace, our online hosting provider, so that they can provide website services to us. This information is also shared and stored on our email account, hosted by a secure provider, for the purpose of responding to your queries and processing bookings. Where a service has been purchased, this information may also subsequently be used for receiving information from yourself and sending reports to you.

North Wales and Chester Dyslexia Assessments keep personal information, assessment reports and relevant documents required to compete the report in line with guidance by Patoss, the professional association of teachers of students with specific learning difficulties, who recommend that:

Records should be kept for as long as the time limit for civil proceedings would apply:

  • Where a service is contractually agreed, for 6 years.

  • If it is a matter of a service contractually agreed, records should be kept for younger learners until they reach the age of 24, or for 6 years after the contract finished for those over 18 at the time of the contracted service.  At the end of this time period North Wales and Chester Dyslexia Assessments will securely dispose your information.
